Your Life That Works assessment can provide important information to share with healthcare professionals, and we encourage you to do so. Whether you choose to share your results with a healthcare professional of any kind — that's your call. We're here to help you understand your starting point. What you do with that understanding is up to you.
This document explains how Life That Works operates, what we offer, what we don't offer, and the relationship between you and us. We've written it in plain language because we believe people deserve to understand what they're agreeing to. If anything here is unclear, email us at support@borntobewell.org and we'll explain it.
By creating an account or using Life That Works, you agree to these Terms. If you don't agree with them, please don't use the service.
1. Who we are
Life That Works (LTW) is operated by Preferred Chiropractic Doctor, Inc. ("PCD"), an Alabama S-corporation. References to "we," "us," "our," and "Life That Works" in these Terms refer to PCD operating under the Life That Works name.
2. What Life That Works is
Life That Works is a wellness screening and education platform. You answer a structured self-assessment about how your body and life are working. We score your responses, return your results as a numeric score with color-coded bands across multiple categories, and provide educational Insights based on what your responses suggest.
Our Insights provide a time-stamped reflection of your self-reported responses, interpreted through current accepted health-related research. They are not diagnoses, not prescriptions, and not predictions. They are a snapshot — a starting point for conversation, reflection, and your own decisions.
3. What Life That Works is not
Life That Works is not a medical service. We are not your healthcare provider. We do not:
- diagnose any condition,
- treat any condition,
- prescribe medications or supplements,
- replace consultation with a licensed healthcare professional,
- guarantee any health outcome.
If you're experiencing severe symptoms, please contact a healthcare professional promptly or call your local emergency number. Do not delay seeking care because of anything you read on Life That Works.
4. Your relationship with healthcare professionals
Life That Works is not your healthcare provider, and we do not stand between you and any healthcare professional you choose to work with. If you take your results to a healthcare professional, that's a relationship between you and them — including any advice they give, any care they provide, any costs they charge, and any outcomes that follow. We're a screening and education tool. We are not in the room with you and your healthcare professional.
We do not share your assessment results with any healthcare professional on your behalf. You may choose to share your results with anyone you wish — that decision and that transmission are yours alone. We are not involved.
5. Eligibility
You must be at least 18 years old to use Life That Works. By creating an account, you confirm that you are 18 or older. Life That Works is not designed for, and should not be used by, anyone under 18.
You may use Life That Works for personal, non-commercial purposes. If you're a healthcare professional or organization interested in using Life That Works in a different capacity, contact us at support@borntobewell.org.
6. Your account
To use most features of Life That Works, you'll create an account. You agree to:
- provide accurate information,
- keep your login credentials confidential,
- be responsible for all activity that happens under your account,
- notify us promptly if you suspect unauthorized access.
You can update or delete your account at any time. See Section 14 for what happens to your data when you delete.
7. Your responses
You are responsible for the accuracy of the responses you provide during your assessment. The more truthful and reflective your answers, the more meaningful your Insights will be.
You agree not to:
- enter knowingly false information to manipulate or test the system,
- impersonate another person,
- use the service to provide assessments for someone other than yourself,
- attempt to interfere with or disrupt the service in any way.
8. Subscriptions, payments, and refunds
Life That Works offers both free and paid services. Free use gives you a snapshot of your overall score and category breakdown. Paid membership unlocks expanded Insights, personalized programs, and additional features.
Billing. Paid memberships are typically billed monthly through Stripe, our payment processor. By starting a paid membership, you authorize us to charge the payment method you provide on a recurring basis until you cancel.
Cancellation. You can cancel anytime through your account settings. When you cancel, you keep access through the end of your current paid period; we do not pro-rate partial months.
Refunds. If you cancel within 30 days of your first paid subscription with us, we'll refund your most recent payment in full — no questions asked. After that initial 30-day window, payments are non-refundable, but you can cancel at any time to prevent future charges.
Price changes. We may change our pricing. If we do, we'll notify you in advance, and any price change will apply only to future billing periods — never retroactively.
9. Intellectual property
We own the Life That Works platform, including its questions, scoring methodology, Insights, content, design, code, and branding. You may not copy, reproduce, redistribute, or create derivative works based on Life That Works without our written permission.
You own your responses and personal information. By using the service, you grant us a limited license to use your responses to provide the service to you, improve our service, and (in anonymized and aggregated form) inform our ongoing research and product development. 10. Acceptable use
You agree to use Life That Works lawfully and respectfully. You will not:
- use the service for any unlawful purpose,
- attempt to gain unauthorized access to any part of the service,
- attempt to reverse-engineer, decompile, or extract source code,
- scrape, mine, or harvest data from the service,
- introduce malware, viruses, or any harmful code,
- use the service to harass, threaten, or harm anyone,
- resell or commercially exploit the service without our written permission.
We may suspend or terminate accounts that violate these rules.
11. Disclaimers
Life That Works is provided "as is" and "as available." We do our best to keep the service accurate, reliable, and available, but we don't guarantee that it will be uninterrupted, error-free, or that it will produce specific outcomes for you.
We make no warranty — express, implied, or statutory — about:
- the accuracy or completeness of any Insight,
- the suitability of Life That Works for any particular purpose,
- any specific health, wellness, or lifestyle outcome,
- the fitness of any general suggestion for your individual situation.
Health is individual. What's appropriate for one person may not be appropriate for another. Use your judgment and consult appropriate professionals for decisions that matter.
12. Limitation of liability
To the maximum extent permitted by law:
We will not be liable for any indirect, incidental, special, consequential, or punitive damages — including loss of profits, loss of data, or personal injury — arising from your use of, or inability to use, Life That Works.
Our total liability to you, for any claim arising out of or relating to these Terms or your use of Life That Works, will not exceed the total amount you have paid us in the twelve (12) months preceding the event giving rise to the claim. If you have paid us nothing in that period, our total liability is zero dollars.
This limitation applies whether the claim is based on contract, tort, negligence, strict liability, or any other legal theory, and even if we have been advised of the possibility of such damages.
13. Indemnification
You agree to indemnify and hold harmless Life That Works and PCD, including our officers, directors, employees, and agents, from any claims, damages, losses, or expenses (including reasonable attorneys' fees) arising from:
- your use of the service,
- your violation of these Terms,
- your violation of any rights of a third party,
- your interactions with any healthcare professional or third party as a result of using Life That Works.
14. Account deletion and data
You may delete your account at any time through your account settings or by emailing support@borntobewell.org.
When you delete your account, we soft-delete your data immediately. For 30 days afterward, your account and data are inaccessible to you but recoverable if you change your mind — just contact us. After 30 days, your personal information is permanently deleted. Anonymized and aggregated data — meaning data that cannot be traced back to you — may be retained indefinitely to improve our service and inform our ongoing research.

15. Termination
We may suspend or terminate your access to Life That Works if you violate these Terms, if we're legally required to, or if we decide to discontinue the service. We will give you reasonable notice when possible, and we will refund any unused portion of a prepaid subscription unless termination is due to your violation of these Terms.
You may terminate your account at any time. Sections that by their nature should survive termination — including disclaimers, limitations of liability, indemnification, and dispute resolution — will continue to apply after termination.
16. Changes to these Terms
We may update these Terms from time to time. When we make material changes, we'll notify you by email or through the service before the changes take effect. By continuing to use Life That Works after changes take effect, you accept the updated Terms. If you don't agree with the changes, you can cancel your account.
17. Governing law and dispute resolution
These Terms are governed by the laws of the State of Alabama, without regard to its conflict of laws principles.
Any dispute arising out of or relating to these Terms or Life That Works will first be addressed by good-faith discussion between you and us. If we can't resolve the dispute through discussion, the dispute will be resolved by binding arbitration in Alabama, conducted in accordance with the rules of the American Arbitration Association. You and we each waive any right to a jury trial and to participate in a class action.
Nothing in this section prevents either of us from seeking injunctive or equitable relief in a court of competent jurisdiction for matters involving intellectual property, confidentiality, or security.
18. Severability and entire agreement
If any provision of these Terms is found to be unenforceable, the remaining provisions will continue in full force. These Terms, together with our Privacy Policy, constitute the entire agreement between you and us regarding Life That Works and supersede any prior agreements.

Email us at support@borntobewell.org. We review messages every day and respond as best we can. There are real limits to how many we can answer, so non-urgent messages may not get an individual reply — but we read them.
